Short answer: only for diabetes, not weight loss. New York's Medicaid pharmacy program, NYRx, does not cover GLP-1s for weight loss — Wegovy, Zepbound and Saxenda for chronic weight management are excluded by state regulation. It does cover GLP-1 drugs for type 2 diabetes — Ozempic, Mounjaro, Trulicity, Rybelsus and Victoza — with prior authorization.
New York's Medicaid weight-loss drug exclusion is written into state regulation (18 NYCRR §505.3), so anti-obesity GLP-1s aren't a covered NYRx benefit. The coverage that exists is a diabetes benefit.
What's covered — GLP-1s for type 2 diabetes
New York Medicaid covers GLP-1 drugs such as Ozempic, Mounjaro, Trulicity, Rybelsus and Victoza for type 2 diabetes with prior authorization. The prior authorization typically looks for a documented type 2 diabetes diagnosis and, often, a trial of a first-line oral drug such as metformin.
Covered another way?
Even though the weight-loss indication is excluded, the same molecules can be covered under a different FDA-approved use:
- Wegovy for cardiovascular risk reduction in people with established heart disease — covered under that indication.
- Zepbound for obstructive sleep apnea — covered under that indication.
- The same semaglutide/tirzepatide as Ozempic or Mounjaro for type 2 diabetes.

- Talk to your prescriber about the diabetes or heart picture. If you qualify under one of those indications, a covered GLP-1 may be an option — a real clinical path, not a loophole.
- Weight-loss surgery may be covered even though the drugs aren't — a separate Medicaid benefit with its own criteria.
