Short answer: weight-loss coverage ended for adults on January 1, 2026. Pennsylvania Medicaid no longer covers GLP-1s for weight loss for adults 21 and older. It still covers them for type 2 diabetes, for members under 21, and for specific non–weight-loss conditions (sleep apnea, heart disease, MASH). Anyone continuing past December 31, 2025 needed a new authorization.

What changed on January 1, 2026

Under a Pennsylvania Department of Human Services Medical Assistance bulletin issued November 24, 2025, Medicaid stopped covering GLP-1s (Wegovy, Zepbound, Saxenda and the diabetes brands used off-label) for weight loss in adults 21 and older, effective January 1, 2026.

Frequently asked questions

Does Pennsylvania Medicaid cover Wegovy for weight loss?

Not for adults 21 and older, as of January 1, 2026. Pennsylvania Medicaid ended weight-loss GLP-1 coverage for adults. Members under 21 can still be covered for weight/obesity with prior authorization.

Does Pennsylvania Medicaid still cover Ozempic for diabetes?

Yes. GLP-1s such as Ozempic, Mounjaro, Trulicity and Victoza remain covered for type 2 diabetes with prior authorization.

Can I appeal the loss of my GLP-1 coverage?

Yes. If you received a denial letter and file an appeal within 15 days of the date on the letter, your GLP-1 coverage should continue during the appeal process.

Are there any weight-related exceptions?

Coverage continues under non–weight-loss indications: Zepbound for obstructive sleep apnea, Wegovy for cardiovascular risk reduction with established heart disease, and Wegovy for MASH liver disease — each with prior authorization.

How we know this: this page is based on the Pennsylvania DHS Medical Assistance bulletin issued November 24, 2025 (effective January 1, 2026) and Pennsylvania Health Law Project guidance, which ended adult weight-loss GLP-1 coverage while keeping under-21, diabetes and other-indication coverage. Medicaid drug rules change frequently and managed-care plans vary — we re-verify this page against the source and date it. Last verified July 7, 2026.
